Years of Experience
We offer care leadership from our cream of leaders with a combined healthcare experience in excess of 50 years. We are leaders in hospice care, home health care, respiratory therapy, medical equipment management, medication management, and care coordination with physicians, pharmacists, and other healthcare support systems. Our hospice services cover the Dallas-Fort Worth metropolitan area and other cities across the State of Texas.
Our Health care Services
We are available to provide personalized and professional care to our clients wherever they call home. Home maybe the private residence of our client and their loved ones. Home may also include Skilled nursing facilities and non-skilled residential facilities, such as group homes and other assisted living facilities.
In-patient Unit
We also provide compassionate care to our clients in stand-alone inpatient hospice units and hospitals, as may be determined by the client and their physician.
Physician Coordination
We provide the needed support for our client and their family to lead the care effort, while we coordinate and implement the physician orders to assure quality care with dignity.
End of Life Management
We believe the end of life is as important as the start of life. We consider it a critical right of patients to have quality care and assurances of dignity in the care delivery. A patient is qualified for hospice end-of-life care as soon as their physician determines a life limiting illness with a life expectancy of 6 months or less.
Medication Management
We work with the client and the physician to administer medications necessary for pain and symptoms management. There is also an informed understanding between the client and their physician that the hospice philosophy does not encourage medications that fruitlessly aim to cure the terminal life-limiting illness. The goal of end-of-life medication regimen is symptom management for comfort and quality of life in the last few months and days of our patients.
Transportation
We encourage our clients to consider the most comfortable means of transportation (whenever transportation is applicable) that will not exacerbate the symptoms being managed.
Client Education
We provide all encompassing client education covering the hospice philosophy, medications, symptoms management, safe use of durable medical equipment, crisis care, and bereavement, among others.
Crisis Care
Our clients’ families lead the care of their loved ones at home 24/7, with our care team visiting and providing the professional guidance from time to time in person and also available by telephone 24/7. We provide continuous crisis care for our clients especially in the event of symptoms such as uncontrollable pain and anxiety that tend to overwhelm family care capabilities, particularly in the last few hours of life.
Comfort Care
Iremia Hospice is an Eden of comfort and peace. Our commitment is to be present and to offer our hand of comfort, peace, and rest, and to provide professional end-of-life compassionate and comfortable care to our clients and their loved ones. We provide care based on our philosophy of peace and comfort.
